Well Water Testing Program Request Form

 

Please complete this 14-question form to request a free private well water test for your childcare facility. Montana Choose Safe Places will review your request and email you to follow up. Each childcare facility is eligible to request one test.

Well water testing through our program is not suitable for water testing to meet DEQ’s regulatory requirements for wells used as a public water supply.
